The Family Clinic is now part of the Colorado Department of Health Chronic Disease Registry which provides for early detection and further prevention from complications arising from chronic diseases. The Family Clinic has implemented this system to capture and track key patient information that assists care team members in pro-actively managing patients with chronic diseases. They include, but are not limited to, diabetes, asthma, cardiovascular disease, and cancer.
